CASE MANAGEMENT

Case Management, also called Care Management, is a process of coordinating and organizing your care through the use of individualized care plans.  Our goal is to ensure you receive the right care and resources, in a timely and efficient manner; this ultimately leads to better health outcomes.

Case Management often involves working closely with patients, as well as their family members, to develop a comprehensive care plan that meets specific needs.  Case Managers may conduct an assessment of a patient’s needs, to create a care plan and coordinate services and resources.

SOCIAL SERVICES

The social services department helps individuals and families manage their health, well-being, and daily living needs by providing patients with resources and support.  These services include:

  • Counselling

  • Financial assistance support

  • Food assistance programs

  • Referrals to community programs

 

The goal of social services is to ensure patients have access to the resources and support they need to manage their health conditions, recover from illnesses, and maintain overall well-being.

HOME HEALTH 

Sometimes we may need a little extra help to make us whole again.  The purpose of home health is to provide medical care therapy and support services in the comfort and convenience of a patient’s home when a medical doctor deems it necessary.  This involves a team of healthcare professionals, including nurses, therapists and other specialists who work together to provide medical care and support to patients in their home.

The goal of home health is to help patients recover from illness, injury, surgery, manage chronic health conditions, and improve their general health and well-being. Home health can include a range of services such as wound care, medication management, physical therapy, occupational therapy, speech therapy and more.
